Our dietitian teaches you how the food you eat affects your health. You’ll get simple tips you can use at home.

We listen to your goals and work with you to create an eating and lifestyle plan you can stick to. We’ll check your progress and help with anything that feels hard.

Diabetes Care

Our registered dietitian is also a certified diabetes care and education specialist and can help you learn about and manage diabetes.

Diabetes Prevention Program

If you have prediabetes, are at high risk for diabetes, or have had diabetes while preganant, this program can help lower your risk and improve your health. A year of live, online calls gives you support with things like:

  • Getting more exercise
  • Healthy cooking
  • Grocery shopping tips
  • Tracking your food
  • Handling stress
  • Eating out
  • Managing cravings
  • Overcoming setbacks
  • Choosing your thoughts
  • Sleeping better

Goals include engaging in at least 150 minutes of physical activity per week and achieving a 5-7% reduction in body weight over one year.

To participate, you must be over 18, have a BMI of 25 or higher, and must not be pregnant. You must also have ONE of the following: A1C blood test between 5.7%-6.4%, fasting blood sugar of 100-125, or diabetes with pregnancy. You may be able to use our sliding fee for this program.

Prediabetes Risk Assessment

It’s important to be screened for prediabetes and take the steps necessary to delay or prevent type 2 diabetes. You can see if you are at risk by taking this prediabetes risk test and talking to your provider about the results.


Providers

We work with you to find ways to pay. We accept Medicaid/BadgerCare, Medicare, and most insurance. We can help you get insurance and apply for our Sliding Fee.

Life can be complicated. We listen and may be able to help with things like rides to appointments, food, housing, family needs, and paying for care.
