Support Beyond the Clinic

Your health is affected by more than what happens during a medical appointment. Transportation, food, housing, family needs, work, finances, and other everyday challenges can all make it harder to take care of yourself.

Community Health Workers

Our Community Health Workers (CHWs) are part of your care team and part of the community. They take time to listen, build relationships, and understand what is happening in your life beyond the clinic.

CHWs work alongside you and your care team to help connect the different parts of life that affect your health. Together, you can identify what matters most, talk through challenges, and create a plan that supports your needs and goals.

Sometimes that means helping you follow through on a care plan. Other times, it may mean finding a way to get to an appointment, talking through a family concern, connecting with community support, or simply having someone who listens and understands.

• Health insurance and coverage questions
• Family or everyday challenges affecting your health

Because CHWs know the communities we serve, they can also help connect you with local organizations and resources when the support you need is outside NorthLakes.
